To create a sustainable, high quality, cost effective care delivery model that connects all or most healthcare components a patient might encounter in order to provide seamless care. This includes primary and specialty care physician practices, hospitals for acute conditions and post-acute care.
Contract with payers and employers to provide health care services with appropriate reimbursement and performance-based incentives related to quality, access and value management. Establish outcomes-based contract relationships that deliver opportunity for enhanced reimbursement either through administrative fees or bonuses (e.g. shared savings) based on mutually agreed upon measures and achievement of targets. To establish value-based contracting arrangements when there are value-driving goals, objectives and opportunity through mutually agreed upon terms. Establish and manage Kettering Health Physician Partners operating budget.
To support the technology objectives and scope for data aggregation, reporting and analytics for Kettering Health Physician Partners.
Focus on clinical optimization by improving value in primary care and subspecialty practice where Value = Quality/Cost. Ensure a fully integrated performance improvement approach with ability to support organizational change through continuous improvement methodologies.
Assure full complement of physician specialists participating within Kettering Health Physician Partners through outreach and engagement with members of the medical staff. Provide a comprehensive schedule of education and outreach to engage participating physicians and their staff with Kettering Health Physician Partners. Obtain delegated credentialing recognition in order to contract with payors.
